The heart of this new model is the Primary Care Coordinator, who is an experienced nurse with a minimum of a BSN. The PCC provides unit-based coordination for 15-20 patients Monday through Friday plus one weekend rotation.
The PCC leads interdisciplinary rounds, care plan development and discharge planning. In addition, the PCC arranges consults with dietary, pharmacy, and therapists and has the authority to decide when a social worker consult is needed.
The PCC visits the patient and her/his family on admission and every day during her/his hospital stay. In addition, the PCC’s business card and photo are displayed in each patient room.
The PCC role evolved from a primary nurse and non-unit based case manager model to a unit-based role for experienced nurses with clinical, financial/discharge planning responsibility. The PCC reports to her/his unit manager.
The PCC model is enabled by the use of new technology, including an online Care Management documentation system that can be accessed from anywhere in the hospital. UPMC Shadyside also uses Voice Care, a voice messaging system where clinical reports are placed using a standard format upon discharge or transfer. Voice Care reports can be accessed by patients, other providers or agencies from their offices, homes or other settings.
